Form of LIC of India

1. Proposal Form for Insurance on Own Life - This form was used to purchase a new LIC policy on one's own life, for a person aged 18 years. This form was revised in December 2017.
2. Proposal Form for Insurance on Another Life - This form was used to purchase a new LIC policy on another life, for a person aged between 0 and 10 years. This form was revised in March 2019.
3. Forms to Surrender LIC Policy - These forms are used to surrender the LIC policy.
4. Proposal Form for Health Insurance Policy - This form was used to purchase a new health insurance policy.
5. Addendum for Multiple Proposals - This form was used to add multiple policies at a time.
